Urgent
- Resuscitated cardiac arrest due to ventricular fibrillation (VF)
- Recurrent sustained ventricular tachycardia (VT) not controlledwith medication
- Recurrent non-sustained VT following MI, left ventricular ejection fraction (EF) <35%

Semi-Urgent
- Symptomatic Wolff-Parkinson-White (WPW) Syndrome
- Recurrent atrial flutter when ablation is considered
- Recurrent atrial fibrillation when ablation is considered
- Atrial fibrillation without control of symptoms or rate
with medical therapy, for AV nodal ablation or pacing

- Asymptomatic WPW Syndrome
- Vasovagal Syncope ACC/AHA HF Guidelines for Device Referral
- Implantable Cardioverter Defibrillator for prevention of Sudden Cardiac Death: Patient has EF <35% w/Heart Failure
- Cardiac Resynchronization Therapy for advanced Heart Failure: Patient on optimal drug therapy w/EF <35% and QRS greater than 120 ms.
